Only current scheduled participants, or their parent/guardian, should use these forms. 

To register as a new participant, use the menu to choose Therapeutic Riding or Horses Helping Heroes, our program for veterans and first responders dealing with post traumatic stress.

Medical Form

After completing the Registration Form, All Registrants must Download the fillable PDF form. This consists of the Emergency Medical Treatment and the doctor’s statement, which must be signed by the participant’s physician. Please scan or photograph the signed form and email it to Program@NewHopeEquine.com 

Report an Absence

Make up lessons will be scheduled only if this form is submitted 24 hours in advance of the date of the scheduled ride and when an instructor, appropriate class, and trained volunteers are available. This allows New Hope to schedule staff, volunteers and horses accordingly.

If you select that you would like a make-up lesson, our staff will be in contact with you. Make-up lessons are only allowed 3 times per year. After 3 absences for any reason, full payment will be required.

Lesson Payment Form

Thank you for your prompt payment to New Hope. If you are paying online, we ask that you use the debit card or bank account to pay from PayPal as this option does not cause New Hope to incur additional fees.

That option does requires a connection from your PayPal account to your bank account. When you reach the “How do you want to pay?” screen in PayPal, you can choose or add your bank account. You may also see a Friends and Family option if on an iPhone, this would work the same way.

Financial Assistance Request Form

New Hope Equine Assisted Therapy

We offer financial assistance to those with incomes under $50,000 annually based on available funds for scholarships. Riders must complete the registration forms including submitting the doctor’s signed form before applying for financial assistance. 

The rider is expected to attend classes regularly. Financial assistance will be revoked for riders who have more than 3 unexcused absences or 3 incidences of not calling to cancel when unable to ride.

You must include a copy of 2 recent paystubs for each working parent/guardian before this application will be considered. Alternate forms of income may be provided in lieu of paystubs such as Award letters for any form of government assistance (including, but not limited to: Unemployment Compensation, Workman’s Compensation, Food Stamps/SNAP, Disability, Social Security, Medicaid, Medicare, Sooner Care, Heartland and TANF).
